Provider credentialing: what it is, how long it takes, and how to avoid delays

You hired a new provider, the schedule is ready, and then you learn they cannot see patients from a major insurer yet. Credentialing delays are one of the most expensive surprises in a growing practice. The process is slow by nature, but most of the extra delay comes from missing information and lack of follow-up.

What credentialing means

Credentialing is how insurance companies verify a provider's education, training, licenses, work history and background before allowing them into their network. Contracting is the related step where the practice agrees to the payer's terms and rates. People often use the words together. Until both are done, claims for that provider may be denied or paid as out of network.

How long it takes

There is no single answer. It depends on the payer, the state and how complete your application is. Plan for several months, often two to four, and sometimes longer for certain plans. That is why credentialing should start as soon as a new provider signs, not when they start.

The best time to start credentialing is the day the offer letter is signed.

Start with CAQH

Many commercial payers use the CAQH ProView profile to collect provider information. Make sure the provider's profile is complete, accurate and attested, and that your practice is authorized to view it. An expired attestation is a common reason applications stall without anyone noticing.

Gather the documents up front

  • State medical license and DEA registration where applicable.
  • Board certification and education details.
  • Malpractice insurance certificate and claims history.
  • Work history with explanations for any gaps.
  • NPI numbers for the provider and the practice.
  • Hospital affiliations or admitting arrangements, if required.

Keep these in one secure folder with expiration dates. Many delays happen when a document expires mid application.

Check every application before it goes out

Names, addresses, tax ID and NPI must match exactly across every form and directory. A small mismatch, such as a suite number written two different ways, can send an application back to the start.

Follow up on a schedule

Submitted does not mean moving. Call or check each payer portal every two weeks. Write down the date, the person you spoke with and the reference number. When a payer asks for something, answer the same week.

Know your effective dates

When approval comes, note the effective date for each payer and share it with your scheduling and billing teams. Scheduling patients for a payer before the effective date is how practices end up with claims they cannot collect.

Do not forget recredentialing

Payers recredential providers on a regular cycle, often every few years. Track those dates and keep CAQH attestations current, usually every 120 days, so a provider is never dropped from a network by accident.

Keep a simple tracker

For each provider and payer, track the status, submission date, last follow-up, next action and effective date. A one page view of this tracker answers the question every practice owner asks: when can this provider start seeing these patients?

When to get help

Credentialing is detail heavy and follow-up heavy. If your office manager is doing it between phone calls and billing, it will slow down. Many practices hand credentialing to their billing partner so it is tracked with the same discipline as claims.

InCareMD supports independent practices with billing and revenue cycle management, including help keeping credentialing on track. If you are adding a provider soon, a free practice review can map out the steps.
