Insurance eligibility checks: the five minute habit that saves your claims

Ask any biller which denial is the most avoidable and you will hear the same answer: eligibility. Coverage that ended, a new plan nobody recorded, a secondary insurance missing from the chart. These claims were never going to be paid as billed. The fix is a short habit at the front desk.

Why checking once is not enough

Patients change jobs, plans renew in January, Medicaid eligibility can change month to month, and people forget to mention a new card. Checking insurance only at the first visit means the information slowly goes stale. Verify before every visit, or at least every month for patients you see often.

What to check

  • Coverage is active on the date of service.
  • The plan and member ID match what is in your system.
  • Your practice and provider are in network for that plan.
  • Copay, deductible remaining and coinsurance for the visit type.
  • Whether the service needs a referral or prior authorization.
  • Any secondary insurance, and which plan is primary.

When to check

The best time is a day or two before the appointment, when there is still time to call the patient if something is wrong. Many practices run checks in a batch each afternoon for the next day's schedule, then handle exceptions by phone. A quick recheck at check-in catches last minute changes.

Five minutes of verification before the visit can save weeks of rework after it.

Use the tools you already have

Most practice management systems and clearinghouses offer electronic eligibility checks. Payer portals fill the gaps. Phone calls are the last resort for unclear results. Whatever you use, save the response or a note in the patient record with the date and who checked.

Tell patients what they owe before the visit

When you know the copay and deductible, share an estimate before the visit. Patients appreciate knowing, and collecting at check-in is far easier than sending statements later. Keep the conversation friendly and simple.

Handle the exceptions quickly

When a check shows a problem, call the patient before the appointment. Ask for their new card, confirm the plan, or offer to reschedule if they need to sort out coverage. A clear script helps your team have these conversations calmly.

Keep the record clean

Scan both sides of the insurance card at every change. Update the subscriber name, date of birth and relationship exactly as the payer has them. Small mismatches cause denials even when coverage is active.

Measure it

Track how many denials each month are caused by eligibility or coverage. As the habit sticks, that number should fall. If it does not, look at which payers or visit types still slip through.

Make it someone's job

Eligibility checks get skipped when the front desk is busy. Assign the daily batch to a specific person, or hand it to a team that does it every day for many practices.

InCareMD front desk and billing teams verify eligibility for independent practices as part of daily work, so problems are caught before the visit. A free practice review shows how many of your denials are preventable.
